This is for community work program. Ideally, I'd like to be able to specify the dates (from/to) and illness (strep throat). I don't have insurance so it's costing me $90 daily, and excessive unexcused absences could mean jail time (@ least a few mo.'s). If not the image or actual document, what software or programs could I use to create one?...
